Neighbourhood Social Prescriber

Social Prescribing is a means of enabling GPs and other frontline healthcare professionals to refer patients to a social prescriber. This provides them with a face-to-face conversation during which they can learn about opportunities to improve their health and wellbeing. The social prescriber can motivate and support individuals to achieve the change(s) that they want to achieve.

Worcestershire Association of Carers has an opportunity for a Social Prescriber to work with health and social care professionals across GP practices situated within Redditch.  The Social Prescriber will be responsible for delivering a person-centred holistic service, supporting people to improve their health and wellbeing by signposting to and connecting with local services and assets in their community

ROLE DESCRIPTION

Responsible to:  Social Prescribing Lead

Base:                   Positions in Redditch General Practice Surgeries.

Pay scale:           £21,745 pro rata

Hours:                 Up to 33hrs per week Mon – Fri. Occasional Saturdays will be required.

                             Fixed term to March 2020

End of Life Carer Advisor

Worcestershire Association of Carers has recently launched their End of Life Service offer to carers who are looking after someone with a life-limiting illness. We now offer intensive support to carers both in a hospital or community setting and are looking to recruit End of Life Carer Advisors to support the service.

Responsible to:  End of Life Carer Lead

Base:                  Post 1.  Alexandra Hospital, Redditch but will work on a flexible basis across Redditch and Worcester Acute Hospital

                           Post 2. Based at Head Office, Polysec House Hindlip Lane,  Worcester but will be out in the community.

Pay scale:           £21,745 pro rata

Hours:                Up to 24hrs per week.

Project Assistant

Worcestershire Integrated Carers Hub provides Worcestershire Carers with information, advice, support and training through a single point of contact via the hub and reaching out across the county through a network of community spokes.

We are looking to recruit an office based Project Assistant to assist with a variety of projects at Worcestershire Association of Carers, primarily across communication, administration and other duties as necessary for the timely completion of projects. The right candidate will be creative, organised and eager to learn.

ROLE DESCRIPTION

Responsible to:  Marketing and Communications Lead

Base:                   WAC Head Office, Polysec House, Hindlip Lane, Worcester WR3 8TJ

Pay scale:          £17,732 per annum (pro rata for part time) - £14,377 actual (30 hours)

Hours:                30hrs per week (Actual hours negotiable)
