Are you looking after a loved one who is nearing the end of their life?

Caring for someone nearing the end of their life can be both rewarding and challenging; probably be one of the most difficult things you will ever do. However, knowing you've done everything you can to make sure your loved one's wishes are respected and that they die with dignity can be very comforting to you and to your loved one.

Caring for someone with a life-limiting illness can be a journey that lasts a few weeks, months or even many years. You may need to consider a range of options in order to fulfil your loved one's wishes and to ensure both you and the person you look after receive the best care possible at the end of their life. Worcestershire Association of Carers has a number of services designed to support carers who are looking after someone with a life-limiting condition.

No one wants to think about getting ill and dying, but having a plan makes it easier. Thinking about supporting your loved one in making a will, deciding what kind of care they would like, or by helping to understand their wishes can make their last days easier for them, for you as well as families and friends.

What information and advice the End of Life service can provide

- Information and advice on the practicalities of caring such as nutrition and equipment.

- Help to navigate health and social care services

- We can provide information about a wide range of useful organisations

- Supporting you with difficult conversations around Advanced Care Planning (ACP)/ Recommended Summary Plan for Emergency Care and Treatment (ReSPECT)

- Legacies around making a will, making a funeral plan, organ donation options and managing digital legacy

- Our experienced staff will have a conversation with you to identify appropriate support to help you in your caring role.
